Recordings and Observations
As part of the supervision of students, sessions may be audio/video recorded, and/or observed through a one-way mirror. Recorded material may be viewed individually by the clinician and supervisor, or by the students in class as part of group training and supervision.
All recorded material is deleted when supervision is completed, and no later than the termination of the assessment. Recorded material does not become a permanent part of the clinic record.
Confidentiality and Privacy Policy
All records relating to clients are confidential and retained in secure files in the clinic. Access to identifying information is limited to clinic staff, the assigned clinician, and supervising psychologists. As part of graduate training, case material may be discussed with other graduate students under the supervision of a registered psychologist. Written material will be presented without identifying information.
Information pertaining to assessments, including written reports, may only be released to other parties with the informed consent of legally authorized persons (usually the persons who gave the initial permission for the assessment). There are exceptions to the confidentiality policy that are required by law. Clinicians must report to authorities when there is suspicion of child abuse, when clients indicate that they may pose a significant danger to themselves or others, when clients report sexual abuse by a health care professional, when the court issues a subpoena for record or testimony, or when clients indicate the abuse of an elderly person in a long-term care facility or retirement home by staff at that facility.
Records are maintained in the Clinic for 10 years after the date they are last seen. In the case of children or adolescents, they are maintained for 10 years after they reach their 18th birthday. For more information about how Clinic files are protected you may consult our Privacy Policy.
